How to Apply for Adaptive Sports Equipment

Attention Athletes! Apply for Adaptive Sports Equipment for anyone 21 years of age or younger residing in the United States.  

Please download and fill out equipment application below. Large or Small items can be approved by our non profit organization. In the application please list diagnosis and sports waiver/release along with needed equipment for that sport/organization.

Sporting equipment might need a sports waiver or release to play along with organization (example: Special Olympics or Challenger Little League etc). 

Apply for Adaptive Sports Equipment.  Supplying any adaptive sports tool or sporting device. From Adaptive clothing, shoes, wheel chairs, pacers, headphones, and much more. No item is too big or too small.
