Please download and fill out equipment application below. Large or Small items can be approved by our non profit organization.

Equipment referral or medical necessity letter from PCP, therapist, or specialist. If it applies to your application-denial letter from insurance.

Apply for Adaptive Medical & Nonmedical Equipment. Adaptive medical or sports equipment, learning materials, adaptive daily tools, adaptive bikes, wheelchairs, crutches, walkers, noise cancelling headphones, and much more. No item is too big or too small.
